St. Matthew's Church Registration Form
This is the form for St. Matthew's Church. Fill it out and send it to the church ASAP.

St. Matthew's Medical Release and Permission Slip
This allows us to get medical attention, God forbid, if it is needed.
Make sure your Tetnus Shot is up to date.

Group WorkCamp Foundation's Registration Form
This is Group WorkCamp's form - fill it out carefully and make sure you and your parents sign everywhere asked.
If your parents do not sign, you not be admitted to the camp. Very important.
